Board Certified Geriatric Pharmacists (BCGP)
Our consultant pharmacist team consists of clinical pharmacists who are highly trained in geriatrics, and fluent in the regulations that affect long term care. Several of our team members have achieved the prestigious designation of Board Certified Geriatric Pharmacist (BCGP), and our entire team belongs to the American Society of Consultant Pharmacists (ASCP). Through continuing education, national meeting attendance, and our involvement with ASCP, we remain up to date on current medical practices and regulatory changes.
Our consultants are available for a variety of educational in-services, QAPI and infection control meetings, and when requested psychotropic management meetings. We can tailor medication regimen reviews to your specific needs. Our comprehensive chart review includes, but is not limited to, identifying drug interactions, contraindications, duplication of therapy, unnecessary medications, excessive duration, cost savings, clinical monitoring, maintaining regulatory compliance, gradual dose reductions for psychotropic medications, appropriate dosing for the elderly and renal impairment, and antibiotic stewardship.
Our team is highly professional and thrives working within an interdisciplinary team. Working with the primary care provider and the facility interdisciplinary team, our pharmacists will ensure each resident’s medications are the most appropriate, most effective, the safest possible, and are committed to preventing medication related problems.
